Application Process

APPLICATIONS WILL BE CONSIDERED BY THE ADMISSIONS COMMITTEE ONCE THEY ARE COMPLETE WITH THE FOLLOWING:

All Applicants:

  • Application
  • Please download and complete the following by hand.
  • Notre Dame Academy Application

    OR
  • Apply Online 
  • Application Fee of $125 per applicant
  • Make checks payable to Notre Dame Academy. Application fees are non-refundable. Those applying online will need to pay online or mail in their application fee of $125 separately after submitting their application via the Web.
  • Verification of Active Church Membership
  • It is the responsibility of the parent/guardian to fill out the top portion of this before sending it to the pastor of the church in which you are currently active. The church should return the form directly to Notre Dame Academy Admissions Office. Supplying a stamped, addressed envelope with the form should help expedite the process.
  • Birth Certificate  A copy is acceptable.
  • Baptismal Certificate (Catholic applicants only)  A copy is acceptable.
  • Current Georgia Immunization Form 3231
  • A copy of any Supplemental Evaluations (i.e., Educational/Psychological Evaluation, Speech and Hearing Evaluation, and Physical or Occupational Therapy Evaluation)

Grades Kindergarten through 8th grade only:

  • Current Georgia Dental/Vision/Hearing Examination Form 3300
  • Previous Schools Transcripts
  • Parent/Guardian must sign and submit a Transcript Release Form to the registrar of the school where the applicant is currently enrolled. Obtaining this transcript is the sole responsibility of the parent or guardian. Please allow time for the school to fulfill the transcript request in advance of the deadline for applications at NDA.
  • Teacher Recommendation Form 1st-8th Grade
  • Teacher Recommendation Form Kindergarten
  • Please give the appropriate Recommendation Form to the applicant's present teacher, who should return it directly to Notre Dame Academy Admission Office. It is the responsibility of the parent/guardian to ensure that this form is completed and submitted to NDA; supplying a stamped, addressed envelope to the teacher expedites the process. Applications will not be considered until the Teacher Recommendation Form has been received.
