Application Process

In order to be considered for admission, the following should be submitted:

All Applicants:

  • Notre Dame Academy Application or Online Application
  • Application Fee of $125. Checks may be made payable to Notre Dame Academy or paid online via credit card.  Application fees are non-refundable. 
  • Copy of Birth Certificate
  • Georgia Medical Form 3231 (Immunization Form)
  • Copy of Baptism Certificate (Catholic applicants only)
  • Active Church Membership Verification Form 

In addition, Kindergarten through Grade 8 applicants must submit:

  • Georgia Medical Form 3300 (Hearing/Vision and Dental Screening)
  • School Transcripts requested from current school via 
Notre Dame Academy Transcript Release Form
 
  • Teacher Recommendation Form for appropriate grade from current teacher

Teacher Recommendation Form Kindergarten or
Teacher Recommendation Grades 1-8

Assessments and shadowing will be scheduled for all applicants in Grades 1-8 who have a complete application on file.

Notification letters will be mailed on April 6, 2012 in accordance with AAAIS guidelines.
