2011-12 School Forms
Forms for current students:
All students must have their immunization records (Form 3231) on file with the school. Students in grades kindergarten through 8th Grade must also have their Hearing, Vision, & Dental information (Form 3300) on file with the school.
The following forms are required to be updated yearly:
- Clinic Information and PE Clearance Form(EYC)
- Clinic Information and PE Clearance Form (E & MS)
- GHSA Physical Form (required only if student is participating in a sport)
- Audio/Video Consent Form
- Carpool /Student Release Authorization
- Science Safety Contract
